Who can avail of Hospice Care Services?

Hospice should be considered whenever someone is in the advanced stages of any of the following progressive illnesses:

• Cancer
• HIV/AIDS
• Kidney failure/End-stage Renal Disease (ESRD)
• Heart disease, especially Congestive Heart Failure (CHF)
• Neurological conditions such as Parkinson’s, ALS (Lou Gehrig’s disease), stroke
• Dementia, including Alzheimer’s Disease, Lewy Body Dementia, Vascular Dementia
• Lung Diseases including Congestive Pulmonary Obstructive Disease (COPD), pulmonary fibrosis

Our Caring Hospice Team

We provide holistic hospice care service through our specialized team to help you, including nurses, home health aides, social workers, chaplains, supervising physicians, nurse practitioners, and others.

Our services include:

• Around-the-clock, on-call support
• Medications to control symptoms and pain
• Medical equipment and supplies, such as wheelchairs, hospital beds, oxygen, and more

You will receive calls from trained Synergy Hospice management to inquire about a patient’s health status and to help ensure they have sufficient medications, equipment, and supplies.
